International Financial Statement

All non U.S. citizens/residents applying in the U.S. are required to complete this form. All non Canadian citizens/residents applying to the Canadian site are required to complete this form.

All international students who would like to enroll in more than 3 classes are required to submit this form before registering.

All F-1 students who would like to waive the insurance provided through Student Assurance Services are required to submit this form before registering.

All F-1 status holders who desire to transfer to City University are required to submit this form.

All F-1 students who would like to enroll part-time or take a vacation quarter are required to submit this form within the normal registration period.

This form will be used to apply for most City University of Seattle International Student Scholarships.
